Overview
The Geriatrics-Neurology Center of Excellence aims to promote brain health, prevention of cognitive decline, and care of older adults with neurodegenerative diseases of the brain. Based at the Zuckerberg San Francisco General Hospital and Trauma Center, the project has established a streamlined clinical workflow for an interprofessional team-- Geriatrics, Neurology, Neuropsychology, and Pharmacy-- to assess and diagnose cognitive decline in older adults who get care in our safety net system.
Project Details
The Geriatrics-Neurology Center of Excellence has established a streamlined clinical workflow for an interprofessional team to assess and diagnose cognitive decline in older adults at Zuckerberg San Francisco General Hospital and Trauma Center. This includes tools specific for multiple educational and linguistic backgrounds, and an assessment of common comorbidities, such as serious mental illness, traumatic brain injury and substance use disorders. Staff and Atlantic Fellows participate in project-related educational experiences, such as didactic sessions, clinical observation, case conferences, and chart review of patient outcomes. Current goals are to expand patient capacity and improve our care planning and social services connection by adding a behavioral health clinician or social worker.
